Condition
Penis enlargement
I am thinking about enlargement and want to know what can really change. Length, girth and appearance: what the procedures change and what they do not.
What can change, and what cannot?
The procedures change the visible length at rest, the girth and the look of the skin. Lengthening does not change the length when erect, because no tissue is added — the tissue that is already there is released. The firmness of the erection, sensation and fertility stay the same. After lengthening the angle changes: the erect penis points lower, because the ligament that is cut otherwise holds the penis up when erect.
This is talked through in advance at the examination, with an estimate for your body. The gap between expectations and what a procedure changes is the most common reason for dissatisfaction after these procedures.
Why does a penis look shorter than it is?
There are two reasons, and they lie in the build of the body, not in the penis itself.
Part of the penis is inside the body. It is held by a ligament attached to the pubic bone. How big this hidden part is differs from man to man.
Fatty tissue above the base. The area above the genitals, the mons pubis (the Venus mound), collects fatty tissue that covers the start of the penis, more obviously when sitting and at a higher body weight. In some men the visible length therefore increases just by reshaping the mound, without any procedure on the penis.
What does the examination involve?
The examination is led by a urology specialist and takes half an hour on average. It is mostly a conversation: what bothers you, since when and what you expect from a procedure. The examination assesses the skin, the amount of fatty tissue above the base of the penis, the scrotum and how much of the penis is hidden inside the body. The first conversation can also take place without an examination, if that is easier for you.
After the examination you get a clear answer on whether a procedure can give what you are asking for. When the penis is of average length and the dissatisfaction comes from comparison, a procedure is not done; then a conversation helps, sometimes with a psychologist too. At the examination nobody pressures you, and you make the decision at home.
Arrange an examination. Nurse Martina will gladly answer your questions and concerns and arrange an appointment. You book a urological examination, without giving a reason. Opening hours are Monday–Friday 8:00–20:00, other times and weekends by arrangement.
What can be done?
| Goal | Procedure | Also, in the same visit |
|---|---|---|
| Greater visible length at rest | Penis lengthening | Girth enhancement and reshaping of the mound are most often done in the same procedure, with one recovery. |
| Greater girth | Penis girth enhancement | Fatty tissue is most often taken from the mound above the genitals, so the mound is reshaped in the same procedure. |
| A build-up of fatty tissue above the genitals that makes the penis look drawn in | Mons pubis reduction | When greater girth is wanted as well, the fatty tissue that is removed is used for the girth enhancement. |
| The skin of the scrotum is loose | Scrotoplasty | When the scrotal skin extends high onto the penis, that angle is corrected in the same procedure. |
Penis lengthening costs €1,800–2,000, girth enhancement €1,000, and both procedures together €2,500–2,800. Mons pubis reduction in men costs €350–1,200, depending on whether liposuction, a lift or both are done. Scrotoplasty costs €500–1,000.
The procedures are done under local anaesthesia with sedation or under general anaesthesia, and you go home the same day. Most men return to work in about a week, some take longer, and sex is possible on average after four weeks. The exact course is decided at the examination.
With girth enhancement, in the first months the body breaks down part of the fatty tissue that was placed, so the final girth is smaller than straight after the procedure. The procedure can be repeated.
Penile curvature and a weaker erection are a different subject. They are covered on the pages Erectile dysfunction (impotence) and erection problems and Penile curvature correction.
Specialist and team
Specialty: urology — examinations, diagnostics and procedures.
When the procedure is done under sedation or general anaesthesia, the anaesthesia is led by a specialist in anaesthesiology, resuscitation and intensive care who works full time at the clinic.
In the operating theatre, the surgeon is assisted by head nurse Martina Šereg, with the help of nurses Kristina Jurešić and Mateja Tomorad, and in recovery the care assistant Mirjana Kovačić is with you. Martina also looks after preparation, scheduling and contact after the procedure.
Who will operate is decided at the examination, based on the findings and the type of procedure. You know the name of the surgeon before the procedure and meet them at the examination.

Frequently asked questions
How much is gained? With lengthening, as much as the part of the penis that is hidden inside the body; this is assessed at the examination and differs from man to man. With girth enhancement, the girth depends on the amount of fatty tissue that can be placed and on how much of it remains after the first months. At the examination you get an estimate for your own body; no figure is a promise.
Does the erection change? The firmness of the erection and sensation do not change. With girth enhancement the fatty tissue is placed under the skin, outside the tissue that takes part in the erection. After lengthening the angle changes: the erect penis points lower, because the ligament that is cut otherwise holds the penis up when erect. This is discussed at the examination, before you decide.
Does PRP (P-Shot) enlarge the penis? No. PRP is a treatment using your own plasma and does not change length or girth. Whether the treatment is right for you and what can be expected from it is decided at the urological examination. There is more on the page Penile PRP (P-Shot).
Can people tell I have had an operation? The cut for lengthening is at the base of the penis, where it is covered by hair, and for scrotoplasty in the midline of the scrotum. With girth enhancement using fatty tissue, the puncture marks are a few millimetres in size.
Is it normal to feel uncomfortable about coming in? Yes, and that is the most common reason men put off an examination for years. For the doctor it is everyday work, and the first conversation can also take place without an examination, if that is easier for you.
Do you also deal with erection problems? Yes. The procedures on this page do not treat a weaker erection. For a weaker erection the first step is a urological examination, covered on the page Erectile dysfunction (impotence) and erection problems.
