Scrotoplasty
With age or after weight loss, scrotal skin can become loose and longer than before. Scrotoplasty removes the excess skin and, if needed, also corrects the angle at which the scrotal skin meets the penis (penoscrotal angle).
What concerns does it address?
How is it done?
The excess skin is cut away and the edge stitched so that the scar lies in the midline of the scrotum, where it is least visible. Correction of the penoscrotal angle is done when scrotal skin extends high onto the penis and shortens the visible length. The volume of the scrotum can be replaced with fat tissue (fat transfer).
In the first days the scrotum is swollen; snug underwear and cold compresses help. The incision is kept dry, and after washing it is dried by patting.
When is the procedure not suitable?
The procedure is postponed if there is active inflammation of the scrotal skin or a fungal infection of the groin, until they have cleared up. If there is chronic swelling of the scrotum (lymphoedema), it is not done until the cause is clarified, because the swelling would stretch the skin again. In men who plan procedures on the testicles or penis, we do scrotoplasty after them, not before. With uncontrolled diabetes, smoking or a blood clotting disorder, the risk for an incision in a moist area is higher, so we plan the procedure with the doctor who manages the treatment.

Anaesthesia
Before a procedure under general or regional anaesthesia, you are seen by the anaesthetist, who works full time at the clinic; the same doctor is also present during the procedure. The anaesthetic assessment and the anaesthesia for larger operations are included in the price of the procedure. What happens before, during and after anaesthesia is described on the Anaesthesia page.
Risks
Occasional — collection of fluid, asymmetry that needs correction.

Specialist and team
Specialty: urology — examinations, diagnostics and procedures.
When the procedure is done under sedation or general anaesthesia, the anaesthesia is led by a specialist in anaesthesiology, resuscitation and intensive care who works full time at the clinic.
In the operating theatre, the surgeon is assisted by head nurse Martina Šereg, with the help of nurses Kristina Jurešić and Mateja Tomorad, and in recovery the care assistant Mirjana Kovačić is with you. Martina also looks after preparation, scheduling and contact after the procedure.
Who will operate is decided at the examination, based on the findings and the type of procedure. You know the name of the surgeon before the procedure and meet them at the examination.
What happens at the examination?
A urological examination has two parts: a conversation about your symptoms, past illnesses and medicines, and a clinical examination. Depending on the findings, an ultrasound is done straight away, in the same visit, if needed. You receive the findings at the examination.
What if the result is not as agreed?
The result of a procedure is assessed at a follow-up visit, once the swelling has gone down and the scar has matured, not in the first weeks. If it differs from what was agreed at the examination, the same surgeon carries out the correction.

Frequently asked questions
How much skin is removed? As much as is needed for the scrotum to sit close to the body while the testicles have enough room and freedom of movement. Removing too much skin pulls tight around the testicles and causes pain, so we remove skin conservatively.
Does it affect fertility? No. Only skin is removed; the testicles and the vasa deferentia are not touched.
Can the skin loosen again? Scrotal skin naturally tightens and loosens depending on temperature and age, so over the years it may loosen a little again. Removed skin does not come back, but ageing goes on.
How much does the result depend on care after the procedure? The larger the procedure, the more it depends on it, but it always does. Healing takes weeks and months and happens almost entirely at home, so the instructions you receive when you go home, about the wound, physical activity and the scar, are part of the procedure, not a recommendation.
