Penile curvature correction

Curvature of the penis in erection may be present from puberty or develop later, most often because of scar tissue in the sheath (Peyronie's disease). When the curvature interferes with intercourse, it is corrected surgically.

What concerns does it address?

When is surgery done?

Surgery is considered when the curvature is stable and bothersome — it makes intercourse difficult or impossible.

How is it done?

The most common technique shortens the longer side of the sheath with stitches (plication), which straightens the penis. With severe curvature or narrowing, the scar is cut out and replaced with a graft; this is a more complex procedure with a greater risk of weaker erections after the operation. Which technique comes into consideration depends on the degree and cause.

Night-time erections may hurt in the first weeks; you can be given medicine for this.

When is the procedure not suitable?

In Peyronie's disease the procedure is not done until the disease has been stable for at least a year, until the pain has stopped and the curvature no longer changes, because an earlier operation could be undone by new scars. It is not done for mild curvature of up to about 30 degrees that does not interfere with intercourse. When there is marked erectile dysfunction as well as curvature, the correction alone does not solve the problem, so we discuss other options.

General reasons for postponing a procedure (inflammation, a clotting disorder, smoking): When is a procedure postponed?

Anaesthesia

Before a procedure under general or regional anaesthesia, you are seen by the anaesthetist, who works full time at the clinic; the same doctor is also present during the procedure. The anaesthetic assessment and the anaesthesia for larger operations are included in the price of the procedure. What happens before, during and after anaesthesia is described on the Anaesthesia page.

Risks

Occasional — loss of length with plication, change in sensation, some remaining curvature.

Specialist and team

Specialty: urology — examinations, diagnostics and procedures.

When the procedure is done under sedation or general anaesthesia, the anaesthesia is led by a specialist in anaesthesiology, resuscitation and intensive care who works full time at the clinic.

In the operating theatre, the surgeon is assisted by head nurse Martina Šereg, with the help of nurses Kristina Jurešić and Mateja Tomorad, and in recovery the care assistant Mirjana Kovačić is with you. Martina also looks after preparation, scheduling and contact after the procedure.

Who will operate is decided at the examination, based on the findings and the type of procedure. You know the name of the surgeon before the procedure and meet them at the examination.

What happens at the examination?

A urological examination has two parts: a conversation about your symptoms, past illnesses and medicines, and a clinical examination. Depending on the findings, an ultrasound is done straight away, in the same visit, if needed. You receive the findings at the examination.

What if the result is not as agreed?

The result of a procedure is assessed at a follow-up visit, once the swelling has gone down and the scar has matured, not in the first weeks. If it differs from what was agreed at the examination, the same surgeon carries out the correction.

Frequently asked questions

Does the penis get shorter after correction? With the plication technique, the length may be reduced by as much as the longer side was longer than the shorter one. This is discussed at the examination.

Does the curvature come back? In acquired curvature, the disease can progress even after correction. It is monitored at follow-up visits.

How is the curvature measured? At the examination, in an erection induced by injection or from photographs you bring from home. The angle and direction of the curvature and the length of the penis in erection are measured, and we choose the technique accordingly.

Will the penis be completely straight? The aim is a curvature that does not interfere with intercourse, as a rule below 15 to 20 degrees. A completely straight penis is not always possible or necessary.

Can the stitches be felt? With plication the stitches can sometimes be felt under the skin as small nodules, especially in the first months. They rarely cause problems; if they do, they can be removed once the scar has taken over the function.

The curvature has also weakened my erection — what comes first? First a urological examination, because Peyronie's disease often weakens erection, and whether only a correction of the curvature is done or something more depends on that.

→ Erectile dysfunction and erection problems

How much does the result depend on care after the procedure? The larger the procedure, the more it depends on it, but it always does. Healing takes weeks and months and happens almost entirely at home, so the instructions you receive when you go home, about the wound, physical activity and the scar, are part of the procedure, not a recommendation.

Who is with me if I stay overnight in the clinic after the procedure? You stay overnight when the doctor judges it necessary and this is agreed with you. You then spend the night in a room in the recovery unit, which has a bed, a television and a bathroom. A nurse is with you all night and a doctor is on call. The overnight stay and night care are included in the price of the procedure.

When should you get in touch immediately?

These are the signs that distinguish a complication from expected healing. If any of them appears, contact the clinic the same day, without waiting for the check-up:

How to choose a clinic and a surgeon?Six things worth checking before you choose a clinic or surgeon
  • Does the surgeon have a licence to practise, and for which specialty was it issued? The licence is issued by the Croatian Medical Chamber (Hrvatska liječnička komora) and can be checked.
  • Who gives the anaesthesia, and are they present throughout the procedure? Regional and general anaesthesia may be given only by specialists in anaesthesiology, resuscitation and intensive care.
  • Where is the procedure carried out, and what does that facility have if something goes differently from the plan? Equipment for emergency interventions and resuscitation, a recovery unit, central gas supply.
  • What is included in the price, and what is paid separately? Anaesthesia, preoperative work-up, check-ups, dressing changes.
  • What happens if the result is not as agreed, and who then carries out the correction? Poliklinika Barbel's correction policy
  • Time to decide. There is no prescribed number of days between the examination and the procedure, because the path to a decision is not the same for everyone. For some one conversation is enough, others want to think it over for a few weeks or hear a second opinion — both are usual. It is just as usual to come for an examination and decide afterwards that the procedure is not for you. The date of the procedure is agreed only once you are sure, and until then nurse Martina and the doctor are available for any questions that come up.

Book a consultation

Head nurse Martina Šereg answers messages. She will gladly answer your questions and arrange the date of your first examination.

The first examination lasts 30 to 45 minutes on average. The surgeon examines the area you are concerned about, asks about illnesses, medicines and earlier operations, and talks with you about what you expect. If a procedure is appropriate and likely to help, you receive a plan: which procedure is proposed, under which anaesthesia, what recovery looks like and what is included in the price. If a procedure is not needed, you will hear that at the examination too. What happens at the first examination?
