Scar revision

Scar revision changes a scar's shape, direction, width or height — it does not remove it. It is done once the scar has matured, most often after a year, unless it is tightening and limiting movement.

What concerns does it address?

Techniques

A wide or raised scar is cut out and stitched again into a thin line. A scar that runs in an unfavourable direction or is tightening is redirected with Z-plasty or W-plasty, so that it is broken up into smaller segments. An uneven or raised scar is smoothed with dermabrasion, and a sunken one can be filled with your own fatty tissue.

→ Dermabrasion

Caesarean section scar and scars from earlier operations

A wide or retracted caesarean section scar is corrected on its own or together with a tummy tuck. Scars from earlier breast operations are corrected together with a procedure on the breast or on their own.

When is the procedure not suitable?

A scar is not revised before it has matured — at the earliest six months to a year after the injury or operation, because until then it changes by itself and some corrections become unnecessary. It is not revised through inflamed skin. If you are prone to keloids, a different approach is chosen, because the incision itself can cause a new keloid.

→ Keloid treatment

General reasons for postponing a procedure (pregnancy, breastfeeding, inflammation, a clotting disorder, smoking): When is a procedure postponed?

Anaesthesia

Before a procedure under general or regional anaesthesia, you are seen by the anaesthetist, who works full time at the clinic; the same doctor is also present during the procedure. The anaesthetic assessment and the anaesthesia for larger operations are included in the price of the procedure. What happens before, during and after anaesthesia is described on the Anaesthesia page.

Risks

Occasional — a new scar that widens again, especially on tense skin.

Specialist and team

Specialty: plastic, reconstructive and aesthetic surgery.

When the procedure is done under sedation or general anaesthesia, the anaesthesia is led by a specialist in anaesthesiology, resuscitation and intensive care who works full time at the clinic.

In the operating theatre, the surgeon is assisted by head nurse Martina Šereg, with the help of nurses Kristina Jurešić and Mateja Tomorad, and in recovery the care assistant Mirjana Kovačić is with you. Martina also looks after preparation, scheduling and contact after the procedure.

Who will operate is decided at the examination, based on the findings and the type of procedure. You know the name of the surgeon before the procedure and meet them at the examination.

What happens at the examination?

The first examination lasts 30 to 45 minutes on average. The surgeon examines the area you are concerned about, asks about illnesses, medicines, allergies and previous operations, and talks with you about what you expect. If the procedure is appropriate and likely to help, you receive a plan: which procedure is proposed, under which anaesthesia, what recovery looks like and what is included in the price. If the procedure is not needed or would not give you what you expect, you will hear that at the examination too.

→ Examinations and diagnostics

What if the result is not as agreed?

The result of a procedure is assessed at a follow-up visit, once the swelling has gone down and the scar has matured, not in the first weeks. If it differs from what was agreed at the examination, the same surgeon carries out the correction.

Frequently asked questions

Can a scar be removed? No — it can be replaced with a better one. Every incision leaves a scar; revision makes it narrower, flatter, paler or moves it into a natural line of the skin where it is less visible.

How do I care for a scar after revision? In the first months silicone gel or strips, massage, no sun for a year. That determines half of the result.

How much does the result depend on care after the procedure? The larger the procedure, the more it depends on it, but it always does. Healing takes weeks and months and happens almost entirely at home, so the instructions you receive when you go home, about the wound, physical activity and the scar, are part of the procedure, not a recommendation.

When should you get in touch immediately?

These are the signs that distinguish a complication from expected healing. If any of them appears, contact the clinic the same day, without waiting for the check-up:

How to choose a clinic and a surgeon?Six things worth checking before you choose a clinic or surgeon
  • Does the surgeon have a licence to practise, and for which specialty was it issued? The licence is issued by the Croatian Medical Chamber (Hrvatska liječnička komora) and can be checked.
  • Who gives the anaesthesia, and are they present throughout the procedure? Regional and general anaesthesia may be given only by specialists in anaesthesiology, resuscitation and intensive care.
  • Where is the procedure carried out, and what does that facility have if something goes differently from the plan? Equipment for emergency interventions and resuscitation, a recovery unit, central gas supply.
  • What is included in the price, and what is paid separately? Anaesthesia, preoperative work-up, check-ups, dressing changes.
  • What happens if the result is not as agreed, and who then carries out the correction? Poliklinika Barbel's correction policy
  • Time to decide. There is no prescribed number of days between the examination and the procedure, because the path to a decision is not the same for everyone. For some one conversation is enough, others want to think it over for a few weeks or hear a second opinion — both are usual. It is just as usual to come for an examination and decide afterwards that the procedure is not for you. The date of the procedure is agreed only once you are sure, and until then nurse Martina and the doctor are available for any questions that come up.
